Golden Buckeye Program

Free savings and discounts for Ohio residents age 60+ and eligible adults with disabilities.

Important Program Change: Cards are no longer mailed automatically to all Ohio residents when they turn 60. You must manually submit an application to receive your card.

Age 60 and Older

If you recently turned 60, you will need to apply for a card at a participating sign-up site (including most public libraries and senior centers) or use the application form link below.

What to bring: Call ahead to verify your chosen site has physical forms on hand, and bring your Ohio Driver License or State of Ohio ID card to demonstrate your eligibility.

Ohioans over age 60 may also use the form below to request a card if their original card has been lost, stolen, or damaged, or if their name has changed or is misspelled.

Age 18–59 with a Disability

Ohio residents between the ages of 18 and 59 who have an eligible disability as defined by Social Security criteria are fully welcome to join the program.

In-Person Required: You must apply in-person at a participating sign-up site (such as any Scioto County Public Library branch). Individuals in this age group are not eligible to use the online submission form.

Request a Golden Buckeye Card

Eligible Ohioans who are current cardholders or age 60 or older may use this official form to request a temporary or replacement card if their card has been lost, stolen, damaged, misspelled, or if they recently turned 60.
